The doctors welcome new patients who live within our practice area. The Windmill Practice serves the areas of the Sneinton, St Anns and the surrounding areas.

If you move out of the practice area you may need to register with another practice.

Register With Us

To register with us, please check you live within our catchment area; you can do this using the map below.

Once you have ensured you live within our boundary, please complete the online ‘New Patient Registration’ form.

  • Please ensure you have completed the online form as detailed as possible to ensure we have all your relevant health information.

Registration forms are also available from Reception at the Practice.


All patients at the Windmill Practice can see any of the doctors practising here or if you prefer you can see the same GP for continuity.

If you would like a health check with a member of the healthcare team, please ring reception two weeks after submitting your registration forms.

Proxy Access

A GP surgery can give someone proxy access so they can help another person manage their GP health and care.

A proxy may be able to act for the person they support, by:

  • ordering repeat prescriptions
  • booking appointments
  • contacting the surgery or speaking to surgery staff
  • viewing test results or vaccinations
  • accessing all or part of the GP health record, to help with health-related tasks and managing health issues

If you’re aged 16 or over, you can ask for someone to be your proxy for any reason. It does not stop you having access to your GP services yourself

If you help someone else with prescriptions, booking appointments or managing their health and care, getting proxy access could make it easier and more convenient

Named GP

All patients are given a named GP within 20 days of registration and told so at the next appropriate time.


  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
  • The named GP works with relevant health and social care professionals to deliver a multi-disciplinary care package that meets the needs of the patient.
  • The named GP ensures that these patients have access to a health check as set out in section 7.9 of the standard GMS contract.
  • The named GP will not take on 24 hour responsibility for the patient, or have to change their working hours.

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
